Acne Scarring:  Hypertrophic scars and Keloids

Hey guys,

Today I shall discuss a particular type of acne scarring that may occur in certain individuals:  Keloids and Hypertrophic scars.

These scars are thickened /raised and are usually pinkish-reddish in color.  They occur when there is trauma and injury to the skin be it from acne, cuts, piercings etc.

A keloid can form on any part of the body but areas that are commonly affected include the upper arm, chest, jaw line and ear lobes (especially after piercings).

Although harmless, these scars can be quite apparent especially in exposed areas and often affects a person’s self esteem.

Though similar in appearance, there are actually some differences between these 2 types of raised scars (see table below):

 

Keloids

 

Hypertrophic Scars
 

Scarring extends beyond boundaries of initial injury in haphazard fashion.

 

Scarring usually kept within and around initial injury boundaries.
 

Strong genetic association

 

No strong genetic association
 

Can continue to increase in size, itch and can be tender at times

 

Does not really change but instead regresses over time

So what can be done about these scars?

There are several treatment options available such as:

  • Pressure dressings
  • Topical treatments such as Silicone gel / sheets: These require months of application and results are modest.
  • Cryotherapy
  • Surgical excision: However, there is a risk of new keloid formation or the formation of a bigger keloid.
  • Lasers
  • Injectable steroid/ cortisone injections

Personally, I find steroid injections to be the most effective non-surgical method in treating these scars.

How is the procedure done?

The steroid solution is injected into the scar via a needle.  It is a quick procedure with no downtime.

How often can I get it done?

I usually space the steroid injections 1 month apart.

How many treatments are required?

Keloids and hypertrophic scars usually require multiple sessions to achieve good results.  I always tell my patients that it is a regime and there is no one time miracle cure for these scars.

What kind of results can I expect?

Results can be seen after 1 session with improvement in results with subsequent sessions.
